Rick Rhino Catibog has posted an episode from the 5/25-5/29/1987 week of Wordplay(which aired 5/26-5/29 and 6/1/1987 thanks to a 5/5 NBC News special report delaying things by one day). Tom's comments indicate it's the first episode of the week,which aired on 5/26. Here's the June 11, 1990 episode of Classic Concentration taped off WESH in Daytona Beach, FLorida, just a couple of weeks before they switched back to the two strikes format, with a car round after each puzzle solved. Airdate gleaned because in the commercial during the 227 rerun at the end of the clip(reruns aired on NBC daytime at the time), a promo for Where's Rodney, an unsold sitcom pilot with Rodney Dangerfield, which was said to air "tonight", aired on 6/11/90, as per tvtango and imdb. www.youtube.com/watch?v=fhA8i6hHDf0

Jeff) from the final month with this format is also in circulation, and that one is from a week later on June 18, based on the WNBC promo for Inside Edition going behind the scenes of Wheel of Fortune. The final month with this format began on June 5 and ended on June 28, six episodes for each of the final three weeks. Apparently, the switch back to two strikes was on June 29, not July 2.

Memory Lane has posted the 9/10/1990 USA airings of Wipeout and Davidson Squares, ending with the first five minutes of the 9/15/1986 USA 9/10/1990 rerun of $25K Pyramid. That was the day Hot Potato returned to USA through 12/28/1990, replacing Wink Tic Tac Dough, in the wake of the Patrick Wayne version debuting in syndication. Wipeout is from the first four weeks of the run, as there are no nameplates on the contestants and no returning champions, both of which debuted on 10/10/1988, and Squares is from 12/14/1987. www.youtube.com/watch?v=7JB196KUdYY

Jkrame75 has Pam Todd Donna as the premiere episode. Perhaps Gametv Canada aired the first 20 episodes out of order, and the show could have aired the first 20 shows out of order on the original broadcast. Wipeout on USA had been on exactly one year(debuted 9/11/1989) at the point of that video.
